Providing your health history at your first visit with Hooper Family Dentistry and updating it regularly allows us to be up-to-date with current health conditions and medications. Medications may affect your dental care, please provide a complete list of your current medications.
New Patient Forms
Important: Online patient forms should only be completed by patients who already have a scheduled appointment. Forms submitted without an appointment cannot be received or reviewed.
We kindly request that patients complete the new patient form prior to their first appointment. For children under the age of 18, please complete the child form.
